Job Description:

SSM Health, a leading not-for-profit Catholic health system, is seeking a dedicated RN-Case Manager to join our holistic, patient-centered care team. The role involves managing a diverse patient caseload, coordinating with healthcare professionals, and ensuring seamless, compassionate, and efficient care delivery across our integrated health system. With a focus on improving patient health outcomes and supporting our communities in the Midwest, our RN-Case Managers play a critical role in embodying our mission of comprehensive service. Assess new case assignments, review patient needs and create detailed care plans. Coordinate various aspects of patient care from admission to discharge. Liaise between patients, families, and healthcare teams to ensure effective communication and understanding of treatment plans. Monitor patient progress and make necessary adjustments to the care plan. Manage documentation of patient care services by keeping accurate and complete records. Comply with healthcare regulations and maintain confidentiality of patient information. Advise patients and their families on health maintenance and disease prevention. Strong clinical skills in a healthcare setting Excellent communication and interpersonal skills Ability to manage multiple cases effectively Strong organizational and time management skills Proficiency in healthcare management systems and digital records Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license Certification in Case Management (e.g., Commission for Case Manager Certification (CCMC)) preferred
