SSM Health, a Catholic, not-for-profit health system, is seeking a dedicated RN-Intake Coordinator to join our team. This role is crucial in managing patient admissions, coordinating with various healthcare professionals to ensure seamless service delivery, and maintaining SSM Health's commitment to compassionate care. The RN-Intake Coordinator will play a key role in assessing patient needs and coordinating their entry into our services, supporting our mission to serve the comprehensive health needs of the Midwest communities. Evaluate patient referrals to determine appropriateness for admission based on clinical criteria. Communicate effectively with physicians, patients, and family members regarding the intake process and status. Coordinate and schedule patient evaluations and admissions. Maintain accurate and confidential patient records. Collaborate with healthcare professionals to ensure a seamless transition into the healthcare system. Participate in multidisciplinary team meetings to review patient assessments, treatments, and planning. Strong clinical judgment Excellent communication and interpersonal skills Proficiency in electronic health record (EHR) systems Ability to manage time effectively and handle multiple priorities Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN license in applicable state Basic Life Support (BLS) Certification Preferred: Certified Case Manager (CCM)
