ACT Nurse (RN) As part of one of our Assertive Community Treatment
(ACT) teams, you’ll provide compassionate, community-based care
that helps individuals experiencing serious mental illness achieve
stability and wellness. In this role, you’ll combine your clinical
expertise with a whole-person, recovery-focused approach—offering
both direct nursing care and support that empowers clients to
thrive in their own communities. About ACT/ACT TAY/STEPS ACT
(Assertive Community Treatment)/ACT TAY (Assertive Community
Treatment for Transitional-Aged Youth)/STEPS (Shape, Teach, Evolve,
Prepare, Support) programs are a way of providing many types of
service to people with chronic mental health disorders. ACT/STEPS
teams provide services directly to people in their community.
People who access ACT/STEPS services receive the wrap-around
psychiatric services, but within the comfort of their own home and
community. ACT/STEPS team members are comprised of professionals in
the areas of psychiatry, social work, nursing, substance use
disorders, supported employment, and peer support so they have the
skills they need to meet the many needs of the people they serve.
The ACT/STEPS team provides these services 24 hours a day, seven
days a week, each day of the year. This is a great opportunity for

passionate about ensuring individuals receive the care and advocacy
they deserve. Key Responsibilities Partner with clients and the ACT
team to develop, implement, and evaluate individualized treatment
plans Administer and manage medications under the direction of the
team psychiatrist, ensuring safe handling and integration into
treatment goals Provide health assessments, education, and coaching
on medication use, mental illness, and co-occurring conditions
Conduct or assist with comprehensive client assessments and
documentation, including treatment plans, progress notes, and
safety plans Support clients in accessing and understanding medical
appointments and treatment recommendations Participate in daily and
weekly team meetings, consultations, and supervision Provide
on-call crisis intervention and support during evenings, nights, or
weekends as scheduled Maintain compliance with all medication,
safety, and infection control protocols (CLIA, OSHA, and agency
policies) Provide services in community-based settings and assist
with client transportation as needed; driving is an essential
function of this position Requirements, Skills, Knowledge and
Expertise Associate’s degree in nursing required QUALIFICATION(S):
WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS • Experience in an
inpatient or outpatient psychiatric setting preferred • A valid
driver's license and agency established minimum automobile coverage
required LICENSURE/CERTIFICATION RN or GN that must pass the MO
NCLEX exam within 90 days of graduation required You’ll Be a Great
